LiveWell at Home is a free service for anyone aged 55 or over who lives in Sheffield. Our aim is to enable older people in Sheffield to live independently in their own home. We offer a range of flexible support on anything related to housing or health, including: support following discharge from hospital; advice on how to maximise income, apply for grants and benefits, and manage debt; finding a new home, including help to move; a 17-point ‘Home Check’; mental health; loneliness and isolation, help to quit smoking and cut down on alcohol.
As we get older things inevitably start to change. Friends and family move on, it’s not as easy to get out and about and sometimes you just need that extra bit of help to get things done.

And all of the support we offer is design to do just that.
We understand that as we get older things can get harder to do, but that's why we're here. We'll set small achievable goals, and work with you every step of the way to reach them.
Our teams are based in local communities across Sheffield. We can meet with you wherever you feel most comfortable, whether that’s at home or somewhere else.
And if you’d prefer to meet us on an evening, or over the weekend, we’re flexible to what works best for you.

- Mental health, with a particular focus on Dementia.
- Ensuring that no-one in Sheffield feels lonely or isolated.
- Help to quit smoking and cut down on alcohol.
