Providing Support to enable Children, Young Adults living with Complex Health Care conditions to lead an ordinary life. Support at home, at work, in the community, attending social activities, in nursery, school & college. Our team are comprehensively trained to support your specific needs.


Description

Caremark provide community support to people living with complex health conditions and disabilities. We support people living in their own home, family home or tenancies with daily living tasks, personal care, respite and help to attend social activities, education, nursery, school, employment and short breaks.

Supporting people with physical and learning disabilities, sensory impairments and complex health conditions, our experienced support workers enable people to lead an ordinary life progressing their Independence.

We support Children, Young Adults and Elderly people across South Yorkshire with a flexible peronalised support plan from 1 hour a week, day or night support, through to 24hr care where required.

Our experience supporting people with a diverse range of conditions and disabilities enables us to bring a holistic approach to developing a support plan with each person that is unique to them.

Description

We support many young children to attend SEND and Main Stream schools. Our support staff are seamless in attending with a child. We can be with them 1 to 1 all day in the classroom or in the backround for routine care needs, personal care, medication regimes, emergencies and support with activities and break times.

We support seveal childen to transport to and from school and provide an after school support function either at clubs, in the communityh or at home until parents/family are available.

Our support workers are comprehensively trained to meet the specific needs of the child. Such as a communication need, we have support workers trained in Makaton that can encourage and support a child in and out of school to develop their understanding and vocabulary. Children with Autism, we have support workers who are experienced in how to suppoprt a child with everday tasks, interests and social inclusion that they may find difficulty with.

Once we receive an enquiry we work with family, education and cirlces of support to understand the person and how we can help them to be the best they can and achieve their aspirations.
