Age UK Sheffield - Independent Living Co-ordination Service

Summary
Help to maintain independent living at home for older people

Description

Sometimes as we grow older, things change, meaning we can't live as independently as we'd like to. Maybe our health or housing situation has changed. Maybe we've lost a loved one, or lost contact with friends and family. Maybe we're struggling to make ends meet.

Our free Independent Living Co-ordination (ILC) service has been evaluated by the University of Sheffield, and it was found that our clients benefitted from:

• Improved physical health
• Improved mental health
• Improved finances in some cases people were £3,000 per year better off
• An average of 9.24 referrals to other services per clients, to local health and social networks such as lunch clubs and community transport.

The service provides up to three months of free support, maximising income levels, advocating for aids and adaptations at home, and providing links to appropriate local groups and activities.

If you or a family member would like more information, just call us on (0114) 250 2850 for a sensitive and confidential conversation.

Our ILC services can support anyone over 50 who needs the service, and we have the following specific projects within the team:

  • A service for people with long-term health conditions, supported by NHS Sheffield Clinical Commissioning Group
  • A service for people living with or affected by cancer, supported by Weston Park Cancer Charity
  • A service for people who need a benefits check in their home, supported by the Big Lottery Fund
  • A service for armed forces veterans or those who did national service, supported by the Ministry of Defence

A registered charity and a company limited by guarantee 5207254.
