Summary
Real Life Options is a support provider for adults with learning disabilities, mental health and other conditions that may require support. We are one of Sheffield's framework providers and have contracts within the council as well as people who chose to fund our support through direct payments or their own finances.


Description

Real Life Options is a support provider who provides supported living and outreach community based support.

We provide support to a range of people who may have conditions such as:

Learning disability

Mental Health

Physical disability

Behaviours that challenge

Autism/ASD

Substance abuse

We offer support to help people live as independently as they can.

All support packages have a bespoke support plan that is tailored to the individual needs of the client.

Staff undertake full training and mentoring with all services they work in.

Notes

Although our office is based in Chesterfield, there are no requirements to come to the office. As the manager and supported leaving lead both live in Sheffeld, we are able to come to you.

 

The office is a council managed serviced office, this means there is a seperate reception team who facilitate the needs of the building. If for any reason, you are not able to contact us, the reception team will be able to take a message and pass that on to us.

The office is open monday to friday 8:30-5pm excluding bank holidays and holiday periods.

Referral Details

Referrals for support can be made via Sheffield City Council or people independently. All referrals will initiate a meeting whereby the needs of the service are discussed as to whether we can facilitate the package of support and for the client to agree that we are a provider they want. There is no obligation during an inital meeting to accept support from us as a provider.
