Description

See the Person can provide care and support to a wide spectrum of people, with a range of needs or conditions, such as:

  • Ageing Well
  • Dementia
  • Learning Disabilities
  • Physical or Sensory Impairment
  • Complex/Multiple Needs
  • Mental Health

Regardless of the level or type of care and support you need, your service will be tailored to your individual needs and wishes, designed to ensure flexibility and control and promote independence.  It’s your voice and your choice.

Our staff will work with you to make sure that you can remain in your own home within your community.  We understand that you may struggle to adjust to receiving care and support, particularly within your own space and sanctuary, and you may fear the loss of your independence.

Our services are all designed to be delivered “with” and not “to” you, and engaging with your social and community networks is key to the See the Person model.

Your care and support will go beyond simply fulfilling practical needs, but will listen to, consider and support your social and emotional needs too.  Regardless of your situation, we take a strength-based approach that helps you to make better use of your own skills and resources, and provide the additional support and motivation where it is needed to ensure that you get to go outside, continue friendships and take part in community life.

You will be able to communicate directly with your team and they are also able to communicate with your family, friends, local community services and other professionals involved in your care and support, such as your GP and Social Worker

The See the Person model ensures that your friendly team of staff is consistent and local, in order to build a warm relationship that values conversation and compassion as equally as the activities required to keep you safe and well, such as:

  • Personal care
  • Skin, health and oral care
  • Condition-specific support
  • Medication and pain support
  • Nutrition and hydration
  • Social support
  • Shopping and domestic activities
  • Activities of Daily Living

Short-Term Services

In addition to ongoing care and support for those individuals that require a permanent service, the flexibility, responsiveness and person-centred nature of the See the Person model means that we are able to offer a range of tailor-made temporary or short-term services. 

Although you may only require or want additional support for a short time, you will still benefit from the advantages of using See the Person, such as connecting you with your family and social circles and coordinating “wraparound” community and health services to regain and maintain your independence.
