Target Housing is a charity and registered social landlord that has vast experience in providing accommodation and support to vulnerable and homeless people who have difficulties sustaining a tenancy. SupportCare also have over 20 years experience of providing supported accommodation to individual’s with mental ill health or learning disabilities, including individuals with complex needs and dual diagnosis. By combining the skills, experience and resources from both organisations Target SupportCare aim to deliver high quality, person centred support and accommodation to support mental health recovery and growth towards independence. We hope to continue working in line with both organisations shared ethos of ‘giving a chance’ to those individuals who may have had difficulty maintaining a tenancy or engaging in support elsewhere.
We provide both long term and short term accommodation in either shared houses or one bedroom flats that are fully furnished.
Service users will be supported through SDS budgets to maintain their tenancy and to develop independent living skills. As well as receiving one on one support, service users have the opportunity to participate in group activities such as gardening group, curry night, snooker tournaments, day trips and overnight seaside breaks.
Outreach support is provided to service users living in the community across Sheffield. One on One support will be provided through SDS budgets with the aim of achieving agreed outcomes of person centred support plans. Support workers are able to use cars where appropriate in order to support service users to achieve goals.
Outreach support is available to new service users living in the community as well as service users who have moved on from our supported accommodation.
Eligibility and referrals
To be eligible for our accommodation or support services you must have mental ill health or a learning disability, you must have an SDS budget in place or have eligible social care needs in order that you can obtain an SDS budget.
We accept referrals from health and social care professionals, other support or accommodation agencies and self referrals. Please see our website for further information and to download a referral form.
