The Community Health Support Team work together to make sure that the health needs of people with a learning disability are met.
Community Nurses
Community Learning Disability nurses are specially trained to work with people with learning disabilities. Some of the things we can help you with are:
- Help you to get healthier
- Giving you information about health problems
- Mental health if you have stress or are worried
- Working on a Health Action Plan
- Health Facilitators work with Community Nurses to support people with their health needs
Occupational Therapists
Occupational Therapists teach skills to help people live as full a life as possible. This can include:
- Getting washed and dressed
- Eating and drinking
- Shopping and cooking
- Using transport
We can also help people with skills such as:
- Talking to people
- Being confident
- Making decisions
We can help people with more profound disabilities by:
- Making eye contact
- Sensory work
- Improving posture
Physiotherapists
Community Physiotherapists are experienced at working with people who have a learning disability and/or physical difficulty
We can help you with:
- Movement and getting around
- Posture
- Balance
- Pain/stiffness
- Sensory awareness
- Fitness
Clinical Psychologists
Clinical Psychologists are trained to understand how people behave, think and feel. We try to help people with learning disabilities and their carers with things they are worried about. For example:
- Feeling worried or scared
- Feeling low, sad or depressed
- Relationship problems
- Getting angry
- Doing things to hurt yourself or other people
Speech Therapists
There are speech therapists in the community teams who are experienced at working with people who have a learning disability. We can help you:
- If you have problems with your eating and drinking
- If you need help with your communication.
