As an organisation we have adopted the ‘Reach’ voluntary standards which are embedded throughout the organisation. 

We believe that over the years supported living has become increasingly misunderstood and has become, a service type, a model rather than a way of supporting people to have real choice and control over their lives.  Phoenix Healthcare Professionals will support you to live the life you want to, where you want to and with who you want to.

We will work with you to develop your tailored support plan that will meet your needs, choices, preferences and wishes.

What we do…………..

Because everyone is different, we want to focus on you as an individual and put you at the centre of everything we do.  Whether your support need is low or enhanced we can work with you, your family, other professionals etc to support you to live in your own home.

We will involve you at every stage of developing your support plan to ensure you feel safe, empowered to live the way you want and where you want and with whom you choose to live with.  We also encourage you to involve family, friends or any other people who are important in your life or who support you.

We know that finding the right care and support for your needs can be tricky, especially when you want to maintain your independence.  That’s why our supported living options are designed around you, where you want to live and what is important to you.

Supported Living – An alternative

Supported Living is an alternative to residential care.  Supported Living will empower you to choose the right amount of support – how and when you need it, so that you can lead an enriching and fulfilling life whatever your level of need is.

We base your support on positive outcomes, developing environments that focus on promoting and encouraging the people we support to fulfilled lives. You will be able to thrive living in one of our Supported Living properties and become a valued member of the community.

 

 Support To Live Your Life

We will work with you to put your support plan together and will focus on your needs, interests, what’s important to you, achievements you want make and the skills you want to develop.  We can support you with, personal care, financial management, daily living activities, finding education/employment and holidays.  All you have to do, is tell us where you would like to live, if you would like to live alone or share a property.  It’s up to you………..

 

Types of Supported Living

Living Independently –

Choosing to live in your own flat/bungalow with the support you need in place.  This could be in one of our established properties, or we can work with you and our housing partners to look for a property that you are happy with.

Sharing Space –

You may want to live with other people, who like you want to live independently as well, but not ready to live alone and have your support plan in place to meet your needs.

You will have your own room but benefit from socialising with other like-minded people, sharing the communal spaces and the cost of household bills.  You may be looking for the best of both worlds with your own apartment and with some shared communal space so you can choose when and if you want to socialise with others or join any activities that may be happening.

 

Whatever you choose, we will support you every step of the way and, support you to live a fulfilled and independent life.

We work in partnership with housing organisations who will along with us, support you with your tenancy, housing benefit, council tax etc as well as Direct Payments if that is how you want to manage your support.
