SCCCC's Placement Support Scheme Proves guidance and support for those looking for suitable accommodation.
There comes a time when we may need to move on to a safe space to live as we grow older. We can help you find the best option for you. We can support you through the process and offer guidance when looking at Care Homes, Assisted Living, and Independent Living Accommodation. We can offer safe transport you and/or a family member to view up to 3 different venues of choice in our wheelchair-accessible vehicle where required. We will liaise with health professionals to ensure a joined-up service, providing the best support.
We take referrals from a wide range of sources including social work teams, intermediate care teams, care coordinators, and community health professionals. We also take self and family referrals.
