Community Integrated Care is one of the UK’s largest health and social care charities. We work in the community, delivering life-enhancing support to over 3,000 people with a diverse range of care needs, including: learning disabilities, autism, and mental health concerns. We can provide care and support to help you maintain your independence and live the life you want. This can include support in your home (whether personal care, medication, or help to manage your home), or to get out and about in the community (developing your social networks, finding training and employment etc). We can support you for as little as a few hours per week, or up to 24 hours per day with overnight support if you need it.
Whatever support it is you need, we will work with you to plan a person centred service, which is built around what is important to you, and for you.

Qualifications and Quality Standards:
Community Integrated Care is an approved provider for Sheffield City Council and is registered and inspected by the Care Quality Commission (CQC). All staff are DBS checked and are either have or working towards Level 2 in Health and Social Care.
Areas Served: Community Integrated Care is a local provider in Yorkshire, delivering services across Calderdale, Kirklees, Wakefield, Sheffield and in nearby Nottinghamshire.
