Social Prescribing in South East Sheffield

Social Prescribing Link Workers (SPLW’s) aim to improve outcomes and experiences for patients, their families and carers by supporting them to unpick complex issues affecting their health and wellbeing.

We connect patients, their families and carers to community groups and help them develop skills, friendships and resilience through a holistic approach.

Social prescribing can help to strengthen community resilience and personal resilience and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities.

Social Prescribing particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which impact on wellbeing. We have a key role in supporting delivery of the Comprehensive Model of Personalised Care.

What We Can Help With:

  • Connecting with the community (Signposting to appropriate community activities/services)
  • Managing Long Term Conditions (This could include appropriate lifestyle advice or signposting to support groups)
  • Cancer prevention & Living with and Beyond Cancer Project (If regularly DNA’s screening appointments, Cancer Care Review etc.)
  • Diabetes prevention (encouraging the uptake of programmes such as DPP)
  • Support with Social Isolation, Low Mood, Stress
  • Linking in with support for Finance, Benefits, Debt advice

Exclusion Criteria:

  • Those who are actively suicidal/ tried to commit suicide in the past 30 days. 
  • Individuals who are dependent on alcohol or drugs. * we are not able to support someone with reducing drugs and alcohol but can support and encourage them with their engagement with services designed to support with this. 
  • People who have a moderate and severe impairment of cognitive function (e.g. dementia); or moderate and severe impairment due to autistic spectrum problems or learning disabilities are best served by specialist services. *we are happy to signpost their carers to support but the referral should be made for the carer not the individual themselves.
  • People who do not want to engage with SPLW's.
