SACMHA Health and Social Care

At SACMHA we have a number of services available for both you and carer(s) that you may have.

Self-Directed Support
Self-Directed Support (SDS) is when:
You are in control of the money you get for your support
You can choose how to spend the money for your support
You can choose things about your life – for example, who you want to get support from
You can get help to do these things if you need it.

Who can you ask about Self-Directed Support. You can ask:
A care manager or social worker
Other people at the council who know about Self-Directed Support
People who support you now
Your friends or family
Someone who already has a Personal Budget
People who work for SACMHA.

Carer's Support
The carer’s service is built on the recognition that whilst caring can be very rewarding it can also be very demanding, placing a strain on you physically and emotionally. If you are finding caring for someone increasingly difficult because circumstances have changed around your health or family commitments, or for any other reason that affects your ability to continue caring, you will need to know what help is available and how to access it.

  • How to get a break from caring
  • How to get your needs assessed
  • How to challenge decisions or complain

Our Carers Support Coordinator can help you with the above questions whilst you are being support to access a range of services, designed to give you a break from caring or provide you with extra support.

Mental Health Support Service
SACMHA is working in partnership with the Sheffield Health and Social Care Foundation Trust. This partnership takes action to improve the experience and outcomes of African and Caribbean patients and carers. 
SACMHA team members work in partnership with ward staff regarding:

Patient advocacy
Patient discharge planning
Support in the community
Access to SACMHA’s Wellbeing Hub
Access to SACMHA’s Social Café
Access to SACMHA’s Carers Group

The SACMHA Team will be present on the wards every Tuesday 10 am and 2 pm.

Community
Supporting the African Carribean community in several ways.
Community Outreach service: A flexible service delivered where you want it when you want it, this could be for a range of support needs such as:

  • Help with managing your mental wellbeing
  • Help with your letters, bills, and other paperwork
  • Help managing your tenancy
  • Supporting you to appointments
  • Support with finding the service you need, or just help and advice, the choice is yours

Support includes: telephone contact with the office, extra support if need­ed e.g. during a crisis, access to social groups, therapeutic groups and ac­tivities.
