NHS Sheffield Place SEND Team

Who are we?

We are a team of nurses and a project officer.  We have knowledge and experience to support families and other professionals.  This is so we can help children and young people with SEND and their associated health matters.

We are a dedicated team.  This means we only support children/young people and their families with SEND.  We also help the professionals around these families.

We are employed by the NHS South Yorkshire Integrated Care Board (often referred to as the ICB).  This means that we are not employed by any specific hospital in Sheffield.

What do we do and how we can help

We look to make sure that the health part of the law for SEND is provided in the right way across Sheffield.

We do this by:

  • Providing advice and support to children, young people and their families
  • Assessing what health needs a child or young person has.  How these needs may affect their day-to-day life and their access to education
  • Working with Sheffield City Council to make plans for these needs to be met. This may sometimes be part of an Education, Health and Care Plan (EHCP)
  • Working with hospitals and health services in our city to provide advice and support.  This is to help them meet their legal duties for children and young people with SEND
  • Working with schools, colleges and nurseries.  This is to make sure they think about a child/young person's SEND needs and how they can be supported
  • Delivering training to different services across Sheffield

You can also ask any of the professionals who are supporting you to make a referral to our team.  They can use the above email address. They refer when there have been attempts already to resolve health issues.  They may have tried a graduated approach or used existing service arrangements.  They will need to provide evidence of your consent to do this.

Teams to support professionals working with a child or young person with additional health needs

Empower education services supporting children with physical health needs through inclusive practices and ensuring appropriate reasonable adjustments in line with statutory duties and legislation:

  • A brief assessment of physical health needs with a focus on access to education.
  • Assistance with the completion of an Individual Healthcare Plan (IHP).
  • Coordination of health advice for cases with complex physical health needs, including the comprehensive completion and input of the IHP.
  • Guidance for educational provisions and partner agencies on meeting statutory requirements for students with medical conditions.
  • Coordination and facilitation of health training tailored to assessed needs.
  • Referral and signposting to additional health services for training and support related to health needs in education. 

Eligibility: Children and young people aged 0-19 (up to 25 with an Education, Health, and Care Plan) living in Sheffield with physical health needs.

 

Where we work: As a developing service, we are currently only fully active in Localities A, B, and E.

 

Consultation available citywide: Consultation and advice sessions are offered citywide. Monthly sessions for education settings interested in receiving advice. Starting in September 2025!

Oversight and case management of individuals with complex health needs, bespoke advice and support for complex cases, including accessing support from the Designated Clinical Officer (DCO) SEND:

  • Management and oversight of the health element of an Education, Health and Care Plan (EHCP) for complex cases

  • Consultation and advice for partner agencies
  • Referral and signposting to additional health services
