Transitioning from the Children with Disabilities Team

Being in transition is when a young person reaches the age of 16-18. This is where a young person gets support to prepare to move from children to adult services.  This is before their 18th birthday.

The Children’s Transition team will work with young people who:

  • are likely to need care/support from Adult Social Care services
  • have a moderate to severe level of need as a result of their disability
  • have an EHCP in place and are transitioning from one service to another

The Transitions team work with young people and their parents or carers to complete a Transitions Assessment. The assessment will cover what support someone might need when they turn 18. It will also look at what can be put in place before this.  This is to try and help with the young person’s independence and their health and wellbeing.

The Children’s Transition Team works alongside the Adult Transitions Team.  When a young person is at a settled point and/or support is in place, they will move from the Transitions teams to an Adult Locality Team.  This is based on which area they live in.

There is no automatic referral process.

Getting a Transitions Assessment

If the child or young person is already receiving care services from the Children with Disabilities Team

The parent or carer can ask the following to refer their young person to the Children's Transitions Team:

  • SNIPS Worker
  • MAST Worker
  • allocated Social Worker

There are 3 children's disability teams that are part of the Children's Disability Service. This is along with the children's Mental Health Team, the SNIPS team and the children's transition team.

If the child or young person will continue to have support needs when they turn 18 and they have an EHCP

The young person or their parent or carer can ask for an assessment.  This is if the child or young person is not already receiving care services from the Local Authority.

The referral would go to the Adult Transition Team.
