A Council-run service to help with social care.

Supports children and young people: 0-18 years

Referral reason: Where social care support is needed due to a child’s disability, where this is the primary need and an assessment is required to consider support provision. 

People involved: Social workers and support workers in consultation with partner agencies.

Offer: The service offers support to children and young people aged 0-16.

Social workers undertake an assessment to inform planning and social care provision.

This will be a holistic assessment that includes information from other professionals working with the family.
